Your child has expressed an interest in being water baptized. Our policy is to obtain consent from the parent or guardian to preform this special ordinance outside of our general membership. You and your family are welcomed and encouraged to attend our service. I certify that my child will be able to participate in these activities including, but not limited to, being baptized by immersion at Legacy Church. By submitting my name below, I give you permission and consent for my child to participate in Water Baptism at your church.
